The caudal approach to the epidural space involves the use of a Tuohy needle, an intravenous catheter, or a hypodermic needle to puncture the sacrococcygeal membrane . Injecting local anaesthetic at this level can result in analgesia and/or anaesthesia of the perineum and groin areas. The caudal epidural technique is often used in infants and children undergoing surgery involving the groin, pelvis or lower extremities. In this population, caudal epidural analgesia is usually combined with general anaesthesia since most children do not tolerate surgery when regional anaesthesia is employed as the sole modality.

The Pain Medicine Service includes the Regional Anesthesia Service and Inpatient Pain Consultation and Follow-up Service, making up a significant portion of the pain-related services offered at Montefiore. The Pain Medicine Service team works directly with physicians in many specialties, including surgery, in treating patients suffering acute or chronic pain. The team is composed of fellowship-trained anesthesiologists, residents and nurse practitioners who perform consultations, follow-up visits and regional anesthesia techniques for more than 100 patients every month.

As mentioned above, there may be more than one spinal structure causing your spine pain.  Sometimes, there are multiple causes for your spine pain or radiculopathy.  In this case, Dr. Lee may recommend alternative treatment options or steriod injections.  For example, if your sciatica/leg pain or hand pain improves with epidural steroid injection but you still have low back pain or neck pain, you may benefit from facet joint injections.  But, whatever the case, you will need further evaluation by Dr. Lee to decide on further treatment.  Remember that there are multiple treatments available now for pain and just because one method yielded partial or no response DOES NOT necessarily mean you will “just have to live with the pain”!

Reta, I know what you mean, oxycontin is way too expensive!! There are many meds. that can be just as effective & even more effective & a LOT cheaper. Everyone is different & what works for one may not work for another. It also depends on what is causing your pain as opioids are not a one size fits all. You need to tell your doc that it is just too expensive & he should have no problem giving you something different. Be Careful of stopping a time release med. like oxycontin that you have been on for awhile as stopping “cold turkey” can be dangerous, so you need to let your doc know asap. I hope all goes well.

Unfortunately, steroid medications also have an ugly side that limits their use. Too many injections with steroids can lead to serious problems in the surrounding tissues. Ligaments and muscles become weakened by long-term exposure to steroids. Additionally, steroids can cause some serious hormonal problems in the body. Because of the temporary and limited benefits of epidural steroid injections, other types of treatments are necessary in conjunction with this quick-fix for neurogenic pain. Even with an injection, one cannot avoid the necessity for regular exercise, good nutrition, and good health. But, those things take time to relieve pain. With spinal injections, a person can buy enough time to get the slower-acting treatments in place. For now, epidural steroids will continue to be a mainstay treatment for neurogenic pain because the realized and potential benefits continue to outweigh the costs and risks.
